Voluntary Attending Physician Application for Professional Liability Insurance (PDF)

HIC sells primary professional insurance, both Claims Made and Occurrence coverage. Primary insurance is available only to voluntary attending physicians at Beth Israel Medical Center, Maimonides Medical Center, Montefiore Medical Center, Mount Sinai Medical Center and New York Eye & Ear Infirmary. Approved applicants will also be automatically enrolled in the free Excess (Section 18) insurance program.

Excess (Section 18) Professional Liability Insurance (PDF)

Any licensed physician in New York State with admitting privileges in a New York State general hospital who carries qualifying primary insurance limits of at least $1.3 million per person/$3.9 million in the aggregate and who completes a New York State-approved risk management course every two years is eligible for additional coverage of $1 million per person/$3 million in the aggregate at no cost. This application should be used by physicians who are not voluntary attending physicians at Beth Israel Medical Center, Maimonides Medical Center, Montefiore Medical Center, Mount Sinai Medical Center or New York Eye & Ear Infirmary.
