How do I report an incident that may lead to a claim?

The process for reporting a claim varies depending on the type of coverage you have.

 

Primary coverage — If you are a HIC insured Voluntary Attending Physician, please contact your hospital’s risk management department.

 

Excess (Section 18) professional liability coverage — If we provide your excess insurance, but not your primary insurance, call 914.220.1816 or simply download the Notice of Claim Form (PDF), complete and mail it to HIC.

 

I received a letter from an attorney requesting the records of a patient who I treated. What should I do?

Primary coverage — When you receive a summons or a request for records from an attorney, or encounter a situation with a patient that may lead to a claim, it is crucial that you consult your sponsor hospital’s reporting procedures before you release any records. Please review Claims.

 

Excess coverage — When you receive a summons or a request for records from an attorney, or encounter a situation with a patient that may lead to a claim, consult your primary insurance carrier’s reporting procedures before you release any records. You must also report the claim to HIC. Please log in, download a Notice of Claim Form (PDF), complete and mail it to HIC. Call 914.220.1816 if you have any questions.

 

How do I obtain a Statement of Insurance for my professional liability coverage?

To obtain a Statement of Insurance for primary insurance, please contact your hospital’s risk management department.

 

To obtain a Statement of Insurance for excess insurance, please fax a letter containing your name and the name of your sponsor hospital to 914.328.3716. Call 914.220.1816 if you have any questions.

When is my quarterly premium due?

Payments for primary insurance owed to HIC are due in equal quarterly installments on January 1, April 1, July 1 and October 1 of each policy year.

 

I received a renewal application. What is it?

HIC sends a renewal application to its policyholders every year. You are required to complete the renewal application with updated, current information. When returned, the renewal indicates your intention to continue your insurance with HIC, but it does not guarantee that coverage will be provided until the application is accepted by HIC's underwriter. What is the duration of excess insurance policies?

Excess insurance is written annually for the policy period from July 1 through June 30 of each year.

 

What is the duration of primary insurance policy?

Primary insurance coverage is written on a calendar year basis for the policy period January 1 through December 31 of each year.
